Parent Consent and Waiver

Permission to participate

I am the parent or legal guardian of the child named above and give permission for my child to attend Christmas Camp at St. Clair Community Church on John Street in Sombra, Ontario. I understand that this is a Christian faith-based experience teaching about God’s plan for the birth of Jesus Christ. I consent to my child taking part in the camp’s indoor and supervised outdoor activities.

Acknowledgement of risk and limited waiver

I understand that participation in games, crafts and outdoor play involves risks, including slips, trips, falls, collisions, minor cuts, exposure to cold or changing weather, and allergic reactions. I have considered these risks and voluntarily give permission for my child to participate. I will explain to my child the importance of following leaders’ instructions and safety rules.

To the extent permitted by Ontario law, I release St. Clair Community Church and its camp organizers, staff and volunteers from claims that I personally may have arising from the ordinary, inherent risks of my child’s participation. This limited waiver does not release claims arising from negligence or intentional misconduct, and does not waive any rights or claims belonging to my child that cannot lawfully be waived by a parent or guardian.

Emergency assistance and health information

If my child becomes ill or injured, I authorize camp leaders to provide first aid within their training and to contact emergency services as needed. If I cannot be reached, I authorize them to arrange emergency medical assessment or transportation for my child. Health professionals will obtain any treatment consent required by law. I understand that the organizers will attempt to contact me or the emergency contact as soon as reasonably possible.

I confirm that the health and allergy information I have provided is accurate to the best of my knowledge. I will notify the organizers of changes and discuss any medication or emergency care plan before leaving my child at camp. I consent to this information being shared with camp personnel and emergency responders as needed to care for my child.
